Caregiver Binder: What To Include (Printable Checklist)
If you help an aging parent or a loved one with health appointments, medications or bills, you are a family caregiver, like about 63 million other Americans (AARP and the National Alliance for Caregiving, Caregiving in the US 2025). Most caregivers end up with information scattered across phones, sticky notes and portals. A caregiver binder puts it in one place you can grab on the way to the ER.
What to include
- One-page medical summary. Name, date of birth, diagnoses, allergies, blood type, primary doctor, preferred hospital, code status. This page goes first.
- Medication list. Every prescription, over-the-counter drug and supplement, with dose, timing and prescriber (see how to keep a medication list).
- Care team contacts. Doctors, specialists, pharmacy, home health, case manager, family helpers.
- Appointment log. Date, doctor, why, what they said, next steps.
- Questions for the next visit (see questions to ask the doctor).
- Symptom and vital signs log. Blood pressure, blood sugar, weight, pain, mood.
- Hospital stays and discharge plans (discharge checklist).
- Legal documents. Copies of the health care power of attorney, HIPAA authorization, advance directive, POLST/MOLST if any (what these are).
- Insurance. Medicare, Medicaid, supplemental and drug plans; ID numbers kept to last 4 where possible.
- Daily routine and preferences. What helps on a hard day; food likes and dislikes.
- Care schedule for siblings and helpers (sharing care).
- Caregiver's own notes, including your own support resources.
Tips
- Put the medical summary and medication list in the front pocket. That's what the ER asks for.
- Update the medication list after every appointment.
- Keep a photo of the key pages on your phone.
- Store full ID numbers securely, not loose in the binder.
Getting help
The Eldercare Locator (1-800-677-1116, a public service of the US Administration on Aging) connects caregivers to local services such as meals, transportation and respite care.
The Family Caregiver Binder includes all twelve sections as a printable workbook, a care tracker spreadsheet and editable templates. Not medical advice.
